Enabling life with dementia

COPE Program

The Care of People with Dementia in their Environment (COPE) Program is an evidence based program to support people living with Dementia and their Care Partners. Research has shown the COPE program has reduced dependence of the person living with Dementia and improved wellbeing of Care Partners and their ability to support the person to remain at home.

How to access

The COPE program can be accessed through a range of funding sources including a Home Care Package, Short Term Restorative Care Program, Chronic Disease Management Plan, NDIS and Private Health Insurance rebates. Please contact us for further details. People can self-refer or a referral can be made by a Health Professional or Care Coordinator using the referral form.

What is involved

The COPE program is delivered over 10 sessions in the home with an Occupational Therapist. It includes comprehensive assessment to identify cognitive and functional capabilities, dementia education, caregiver skills and stress management. Together the Care Partner and Occupational Therapist will develop strategies to address three main areas of concern for the Care Partner and an activity strategy to enhance meaningful engagement for the person living with Dementia.

Let’s work together

Collaborate Occupational Therapy is based in Perth and was created to work together with people living with Dementia and their Care Partners. This means getting to know a person, identifying strengths and challenges and enabling their best ability to function in everyday life. Collaborate Occupational Therapy are proud to offer COPE, an evidence based program showing the effectiveness of working with Care Partners in supporting people living with Dementia.
